This one gave some further food for thoughts, while describing some very unusual situations where risk taking, and induced errors, has been reduced to its minimum. Among the key words to bear in mind pop up: devil's advocate, reversed and/or removed hierarchy, check lists... A must read book for whoever leads a project, a team...
